Erasing Ten Years Really Is Possible!

As the pictures indicate, erasing years really can be done. Quite easily. Here’s what we did: Fine lines and wrinkles:This lady benefited from starting a product that would turn her skin cells over faster, increase collagen production, thicken her skin, and increase the vascularity (nourishment) and health of her skin. We began with a 0.025% Retin-A (tretinoin cream). Retinoids are the only skin treatment that can achieve such a super anti-aging effect. After a tube of that, we moved to the Skin Medica Tri-Retinol Cream followed by the the Skin Medica TNS Ceramide Treatment Cream.
Volume Loss: Even though she’s only 38, she had significant volume loss and muscle distortion. While there has been no significant weight loss or gain, she had previously been a smoker and a sun worshiper. Adding some Perlane and Restylane in appropriate areas (upper cheeks, naso-labial folds, marionette lines) gently added the look of youth and vitality.
Forehead Lines: We used both Botox and Dysport in her forehead and crow’s feet. Both work wonderfully!
For Uneven Skin Tone: We used Intense Pulse Light followed by Laser Genesis (Nd:YAG Laser) to entire face. This is the single best skin treatment for uneven skin tone.
Slight Lip Enhancement: Using 1 syringe of Juvederm Ultra, her lips instantly became a pretty perky pucker.
Get Carded! The best news? After her filler (and bangs), she went to Chili’s and was carded. That apparently had not happened in years and her elation continues today!

IPL Reverses The Signs Of Sun
We all wanted sun in our previous lives. Many felt that the greater their tan, or even their sunburn, the more fun they had aquiring it. Based on many patient testimonies, this appears to be universal, as it was with me. I grew up in Wisconsin, which meant if you had a sunny day, you had to plot mysteriously to obtain your desired (and deserved!) time in the golden glow. I do confess that their were times in my early twenties, before my later derm schooling, that I would actually call in sick (though it was rare, maybe once a season) if their was a bright, sunny day predicted. Yes, now as more and more patients present with this affliction, I can’t believe I did that, but even more so, many members of my family did it too, though carried it much further.
My beloved cousin Tami loved to lay in the sun. As a young mother and one who loved the summer (short as it was), she bought a time-share in Florida, in order to avail herself to the consistent warming illumination that would produce a skin color change that made it look like one had a great vacation. She really did have great vacations doing this, but never considered the consequences. Ever.
Fast forward to now, when this insatiable desire left her to look older than she was, or at least older than she needed to look given her relatively young age. Who wants to look older? Plus, due to her Irish heritage, she had precancerous lesions showing up that I needed to make sure would go away. Therefore we had: sun damage, and big sundamage (precancerous stuff). Therefore, Levulon.
She tolerated it well (that is to say she did not jump off the table and hail a cab [which would be no avail in South Florida anyway] nor did she retreat to the retreat (an actual room in our facility) in which she could have sought refuge.
Our literature and experience gave her five days until she could go in public and look normal. On day five, I texted her to ask how she was doing (back in the frozen tundra by now) she stated on 9/28/10 that her face was “dry, peeling, itchy, and ugly!” Oh well, I let her know better things were coming. I will show the before and afters once she is finished. I will see her in two weeks. Stay tuned.

Best Sunscreen and Reapplying
Dear Dr. Bonnie,
In regards to your comment about sunscreen. Is there one in particular that you would recommend people to use in the summer? Preferably one that doesn’t decrease efficacy after only a couple hours. Is it true that you need to reapply sunscreen every 2 hours? Are there any sunscreens that last longer and can be worn under make up all day?

Dear Miranda,
I really prefer something that is a physical blocker and most often use products from Fallene, specifically TIZO for the face. TIZO is a non-chemcical based sunscreen that contains Titanium di-oxide, Zinc oxide, and Iron. These physical blocks provide long-lasting protection (you really have to wash this stuff off, as it is extremely water and sweat resistent). That said, NO sunscreen can protect you the same way the indoors can. Or clothes. In other words, sunscreen or sunblock is NOT the same as not being out. But we want to be out, so choose smartly. Since the physical blocks do not break down in the heat, light, or vehicle, they are your best bet.
Why does one need to re-apply sunscreen? One reason is that if you are getting continued sun, you are counting down the numbers of the SPF (whatever it is) as well as the UVA blcoking potential. In other words, if a sunscreen boasts an SPF of say 30, then potentially you can be out 30 times longer with the same amount of exposure as if you were wearing nothing. However, you also have incidental removers, such as scratching an itch, giving a hug, wiping your hair out of your face, etc. These remove the protective ability. Finally, reapplying can add “layers” that may be helpful to increase the physical barrier. It’s worth it.

Sunscreen May Prevent Skin Cancer
Hopefully not many of us have abused ourselves the way this woman has. While people are becoming more educated regarding sun exposure, there still are many who do not understand what constitutes safe sun. Almost everyone is in the same boat when it comes to choosing what is the best sun protection for you and your family. According to Rebecca Sutton PhD, an environmental chemist and a graduate of the University of California, Berkeley, titanium dioxide and zinc oxide, cover a huge amount of the bad light spectrum (UVA and UVB). These are the only two ingredients I ever recommend for my patients, my family, and myself.
Skin Cancer Warning Signs
By the time one starts thinking of skin cancer, they probably have many other signs and a past history indicating that they may have something going on. Frequent burns as a child and a teenager (actually, 3 or more) puts one at much greater risk. Obviously, frequent sunbathing or using tanning beds contributes to the development of skin cancer. Heredity plays a part. Once into the fourth and fifth decades, the skin has attained the appearance of a blotchy, rough, slightly reddened or maybe even yellowish (actinic bronzing) texture and presentation. There may be rough patches that become dry and scaly, then get better, only to become dry and scaly again. These are known as Actinic Keratoses, or pre-cancers.
Your lips may be dry and peel, no matter how much lip balm you apply. Some of these dry, scaly patches may even start to raise and crust. This is known as Actinic Cheilitis, and is also known by other names such as solar cheilitis, farmer’s lip, and sailor’s lip. Personally, living in South Florida and having so many folks that do develop this I feel it should be termed “Tennis Babe Lip”. Treatment (and you do want to get treatment as it is impossible to predict if it will develop into squamous cell carcinoma or not) involves creams, freezing with liquid nitrogen (for small lesions), surgical removal, or laser therapy.
Why Zinc And Titanium?

Zinc Oxide
Zinc and titanium do not degrade like chemical sunscreens or sunblocks, and they are resistant to light and heat, which, wouldn’t you think, would be a necessary component to a sound sunscreen? Not always so. Avoid the chemicals. The physical blocks (again, zinc and titanium) can handle what we throw at them: Heat and Sunlight!
Don’t worry about looking pasty. Today’s formulations are microfine and can be transparent. There is nothing better for your skin’s protection!
The Tanning Bed Base Tan

This does nothing for you other than causing more damage. I have friends who own tanning salons, and I respect their business desire. However, I cannot ever recommend this type of recreation. It is neither healthful nor protecting. It simply makes you look old way before your time, and your dermatologist will have a much busier time keeping up with keeping you healthy. you may not worry about looking youthful now, but you soon will.
